The CSB revealed that the 8-inch pipe that failed had been there for decades and over time it had lost over 80 percent of its thickness. The CSB says Chevron failed to follow its own inspection guidelines.
The CSB said there is no evidence the pipe had ever been inspected.
The plant manager was at the meeting and read a pre-prepared statement but would not answer questions about why the pipe had not been inspected.
The Chevron spokesperson says the company is reviewing its inspection records.
