Chevron crews may have contributed to refinery fire

Chevron's own firefighters may have contributed to last summer's massive refinery fire in Richmond, investigators say.

According to the San Francisco Chronicle, firefighters may have accidentally punctured a main pipeline while tearing away insulation.

Investigators report the pipeline appears to have been penetrated from the outside.

That puncture may have sparked the fire that could be seen for miles across the Bay Area.
