Maggie Gomez is due in court December 1st to answer the felony fraud and perjury allegations.
Prosecutors say she filed an injury claim in 2005 while working as a PR manager at Seton Medical Center.
The DA's office contends Gomez lied about her physical condition to company representatives over two years.
She has yet to enter a plea to the charges.
