A report compiled by CPUC staffers found numerous safety problems on Muni's light rail system. Inspectors also accuse Muni officials of being chronically unresponsive to violations.
Muni official quickly disputed the claims and say they feel blindsided by the allegations.
The report lists eight major safety violations, including problems with defective track, as well as tunnel control systems that operate poorly or not at all.
