Authorities are searching for an at-risk 57-year old man who walked away from a rehabilitation center Tuesday.

Oscar Flores was last seen around 6 p.m. Tuesday, Jan. 20, leaving Kaiser Permanente's Vallejo rehabilitation facility. Officials say due to his medical condition he may have difficulty communicating.

Flores is described as a 57-year old Hispanic male, 5'7" tall, weighing 170 pounds. He was last seen wearing dark gray sweatpants, a black sweatshirt and a black ball cap.

Rehabilitation center patients have the right to leave the facility, but medical personnel and the patient's family and friends are concerned for Mr. Flores' welfare due to his medical condition. They are asking the community for their help in locating him.

Anyone who sees Flores is asked to call 9-1-1 or Kaiser Permanente at 1-707-651-1000.
